**Responsibilities:**

- Provide skilled nursing care to home health patients under the direction of a registered nurse
- Conduct regular patient assessments and document observations accurately
- Administer medications and treatments as prescribed
- Assist with wound care and dressing changes
- Monitor patient progress and report any changes to the healthcare team
- Educate patients and their families on proper home care techniques
- Collaborate with the interdisciplinary team to ensure comprehensive patient care
