Are you a dedicated Licensed Practical Nurse (LPN) with a passion for providing high-quality home health care? Sunbelt Staffing, a nationwide healthcare staffing agency, is seeking a skilled Home Health LPN to join our team for a 13-week contract assignment with a reputable nationwide home health agency in Omaha, NE.

Position Details:

  • Location: Omaha, NE
  • Contract Duration: 13 weeks
  • Hours: Full-time, 40 hours per week, Monday to Friday, 8 AM - 5 PM with occasional weekends and on-call duties (typically one weekend per month).

Compensation and Benefits:

  • Competitive hourly pay (negotiable based on territory budget and experience).

Qualifications:

  • Active LPN license in the state of Nebraska.
  • Active BLS certification from the American Heart Association or American Red Cross.
  • Minimum of 1 year of home health experience.
  • Proficiency in Homecare Homebase software.
  • Reliable transportation and willingness to travel within the assigned territory.
  • Successful completion of Joint Commission onboarding requirements.

Key Responsibilities:

  • Provide compassionate and professional nursing care to patients in their homes.
  • Utilize Homecare Homebase for electronic documentation and patient management.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications, treatments, and other prescribed interventions.
  • Educate patients and their families on disease management, treatment plans, and preventive health measures.
  • Collaborate with interdisciplinary teams to ensure coordinated patient care.
  • Maintain accurate and timely documentation of patient care activities.
  • Participate in on-call rotations and provide weekend coverage as required.
