We are looking for Full-Time, Part-Time, and PRN LPNs to work a contracted role in Penn Yan, NY. Under the direction of licensed nursing personnel, the Licensed Practical Nurse (LPN) performs direct and indirect resident care within the scope of practice as defined by the licensure. The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. This includes understanding the principles of growth and development across the lifespan and the ability to assess data reflective of the patient's status to identify their requirements relative to age-specific needs, providing necessary care to adult and geriatric patient groups.

Contract Details:
Contract Length: 13 weeks
Shift Lengths: 8 hours, 10 hours, or 12 hours
Shifts Available: Day, Evening, and Night shifts open

Job Functions:
Medication Administration: Administer medications to residents as prescribed, ensuring accuracy in dosage and timing. Monitor residents for adverse reactions and report any concerns to the supervising nurse.
Patient Care: Provide basic nursing care, including wound care, catheter care, and assistance with activities of daily living (ADLs) such as hygiene, grooming, dressing, toileting, and incontinence care. Complete documentation as required.
Environment and Safety: Maintain a clean and safe environment for residents, promptly respond to call lights and alarms, use safety mats and low beds, follow procedures for restraint use under nursing direction, and prevent falls and other accidents. Ensure all documentation is completed as required.
Nutritional Needs: Ensure residents' nutritional needs are met according to their care plans, including meal delivery, tray preparation, feeding, and honoring food preferences. Complete necessary documentation.
Rehabilitative Care: Support residents' rehabilitative needs as per their care plans, including range of motion exercises, ambulation, transfers, and proper use of assistive devices. Document all care provided.
Resident Rights and Communication: Report any resident rights concerns, refusals, or other issues to the Charge Nurse or Supervising Nurse promptly. Report any changes in resident status, including physical, emotional, and psychosocial changes.
This role anticipates occupational exposure to bloodborne pathogens, involving potential contact with blood, body fluids, or tissues.

Required Qualifications:
Must live 30 miles away from the facility
Must have at least 1 year of LPN experience
Must have an active LPN licensure in the state of NY
AHA BLS
