Contract Registered Nurse Home Health Job in Johnstown, Pennsylvania

**Requirements:**

- Valid and current Registered Nurse (RN) license in Pennsylvania
- Minimum of 1 year of experience in home health care
- Proficiency in Homecare/Homebase and Oasis documentation
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a multidisciplinary team

**Responsibilities:**

- Conduct comprehensive assessments of patients' health conditions and develop individualized care plans
- Provide skilled nursing care, including medication administration, wound care, and patient education
- Monitor patients' progress and update care plans as necessary
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and effective care
- Educate patients and their families on disease management, medication compliance, and self-care techniques
- Document all patient interactions, assessments, and interventions accurately and promptly

**Contract Details:**

- Position type: Full-time, permanent
- Shift: Monday to Friday, 8:00 am to 5:00 pm
- Hours per week: 40 hours
