Contract RN Case Manager Hospice Role - North Kingstown, RI

Setting: Hospice

Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Contract Length: 13 weeks

On-call: Occasional, 1–2 times per month as back-up

Territory: North Kingstown/South County/Westerly

Office Location: 1130 Ten Rod Rd, Suite A205, North Kingstown, RI 02852

Weekly Mileage: Approximately 100–200 miles

Weekly Productivity: 20–25 units

EMR: Pointcare/HCHB

Responsibilities:

Deliver and coordinate hospice care per physician orders and agency guidelines

Conduct in-home patient assessments and monitor changing conditions

Complete and update patient Plans of Care every 60 days

Supervise and evaluate Hospice Assistants and LPNs

Maintain accurate documentation and communication with all care team members

Qualifications:

Active, unencumbered RN license in Rhode Island or compact state

Minimum of 1 year nursing experience; hospice experience preferred

Current CPR certification

Reliable transportation, valid driver’s license, and required liability insurance

Comfortable using EMR systems and general computer applications
