Title: Per Diem Nurse Practitioner (NP) - Home Health Le Ray, NY & Jefferson County) - Flexible Schedule

Description:

  • Are you a Nurse Practitioner seeking a flexible schedule and the opportunity to make a difference in patients' lives?
  • Sunbelt Staffing is seeking a Per Diem NP to conduct home health risk assessments for Medicare Advantage plan members in Le Ray, NY and surrounding Jefferson County. (Watertown, Adams, and Fort Drum)
  • This is a great opportunity for a compassionate and independent NP to work on-site with patients in the comfort of their homes.

    Responsibilities:

  • Conduct comprehensive annual home health risk assessments (45 minutes average).
  • Evaluate patients' medical history, conditions, and medications.
  • Educate patients on health conditions and medications.
  • Identify gaps in care and provide recommendations.
  • No creation of treatment plans or prescribing medications required.

    Work Schedule:

  • Flexible! Create your own schedule with shifts available Monday-Sunday.
  • Minimum weekly or bi-weekly hourly commitment.

    Qualifications:

  • NY Nurse Practitioner License.
  • Current CPR certification (BLS).
  • Preferred: 6 months of current NP experience.
  • Preferred: Board Certification through AANP or ANCC.
  • MSN degree from an accredited school of nursing.
