Full-Time Nurse Practitioner (NP) Contract | Medford & Grants Pass, OR

Sunbelt Staffing is partnering with a respected insurance provider to hire a dedicated Nurse Practitioner for an exciting direct patient care contract opportunity in Southern Oregon. This role is part of the Institutional Special Needs Plan (ISNP) program, designed to deliver comprehensive, proactive care to patients residing in skilled nursing and assisted living settings.

Contract Details: 

Location: Covering facilities across Jackson and Josephine Counties

Contract Length: 7–12 months

Schedule: Monday–Friday, 8:00 AM – 5:00 PM | 40 hours/week

Dress Code: Business Casual

COVID-19 Vaccination: Required

Setting: Skilled Nursing Facilities and Assisted Living Facilities Position Highlights: 

Work across 3–4 facilities in Medford and Grants Pass, with potential for range throughout the contract

Provide monthly health assessments (or more frequently, if clinically necessary)

Aim to prevent avoidable hospitalizations and promote smoother transitions to home settings

Collaborate with RNs for seamless care coordination

Prescribe/recommend non-narcotic medications in partnership with primary care providers

Identify and recommend updates to care plans to address gaps in care (STARS, HEDIS, COHPS, etc.)

Attend facility rounds, participate in care team meetings, and support advance care planning Position Duties: 

Conduct routine patient visits and assessments

Review residents’ conditions and assist in achieving care goals

Participate in interdisciplinary care team (ICT) communication and care coordination

Attend meetings and report updates on behalf of the NP

Engage with patients, families, and facility staff to ensure high-quality care delivery Qualifications: 

Active NP license in Oregon

At least 2 years of Nurse Practitioner experience, in long-term care, home health, or a similar setting

National certification (AANP or ANCC)

Active BLS through AHA or ARC

Strong communication skills and a collaborative approach

Comfortable navigating technology for documentation and communication
