Fully Remote Home Health Registered Nurse (RN) Contract Opportunity - New York State

Conduct virtual health assessments and Caregiver evaluations.

Develop and monitor personalized care plans based on clinical findings and patient goals.

Coordinate services and transitions of care with the broader healthcare team.

Use remote monitoring and patient self-reporting tools to manage chronic disease.

Provide health education and promote use of community-based resources and in-network providers.

Monitor outcomes and adjust care plans as patient conditions evolve.

Qualifications:

Active, unrestricted RN license in NY; compact license preferred.

BSN required; MSN or graduate-level degree in healthcare preferred.

3–5 years of clinical experience with a strong background in care management, home health, or hospice.

Ability to treat patients across the lifespan.

AHA BLS certification required.

Strong communication skills and comfort with virtual care technologies.

Reliable internet connection and a private, professional remote workspace.

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.

Preferred Qualifications:

Prior case management experience in a health plan or telehealth setting.

Familiarity with Medent EHR or similar systems.

Bilingual proficiency is a plus.

Willingness to obtain Autonomous Practice status in NY if eligible.
