Job Description:

Key Responsibilities:

  • Perform comprehensive patient assessments and create individualized care plans
  • Deliver skilled nursing care, including wound care, IV therapy, and medication administration
  • Ensure timely and accurate OASIS documentation using Homecare Homebase
  • Provide patient and family education on self-care and disease management
  • Participate in on-call rotations as required

    Qualifications:

  • Active RN license in Georgia
  • Minimum of 2 years of recent home health experience
  • Proficiency in OASIS and Homecare Homebase required
  • Current BLS certification (AHA)
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
