Job Title: Home Health Licensed Practical Nurse (LPN)
Location: Agawam, Massachusetts

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is seeking an experienced Home Health Licensed Practical Nurse (LPN) for an assignment in Agawam, Massachusetts. In this role, you will deliver high-quality patient care in a home health setting, manage a caseload effectively, and maintain precise documentation using Homecare Homebase. This position is open to both local candidates and travelers, offering a rewarding opportunity in a supportive and historic community.

Key Responsibilities:

Provide nursing care to home health patients in alignment with their care plans.

Administer wound care, medications, and patient education.

Monitor and evaluate patient progress, updating care plans as necessary.

Document patient care accurately using Homecare Homebase.

Collaborate with a multidisciplinary team to ensure comprehensive care.

Qualifications:

Active LPN license in Massachusetts or compact state licensure.

A minimum of 3 years of LPN experience, including at least 1 year of recent home health care experience.

Proficiency in Homecare Homebase required.

Current BLS certification.

Reliable transportation and valid driver’s license.

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
