Home Health RN - 13-Week Contract - Danville, KY
Job Description:
We are seeking a skilled and dedicated Home Health Registered Nurse (RN) for a 13-week contract in Danville, KY. The ideal candidate will provide high-quality, patient-centered care in the home setting.
Key Responsibilities:
Deliver skilled nursing care to patients in their homes.
Conduct patient assessments and develop individualized care plans.
Perform wound care, administer medications, and educate patients and families.
Complete accurate and timely OASIS documentation.
Coordinate with the healthcare team to ensure comprehensive care.
Requirements:
Active KY RN license.
Minimum of 1 year of recent home health nursing experience.
Proficiency in OASIS documentation is required.
Experience with HomeCare HomeBase (HCHB) preferred.
BLS certification.
Strong communication and organizational skills.
