Job Details:
Location: Vernon Hills, IL
Contract Length: 13 weeks (with potential for extension)
Shift: Days, 8-hour shifts
Hours per Week: 40
Caseload: Varies, with visits typically scheduled within a reasonable radius
Documentation: OASIS proficiency is absolutely required; experience with HomeCare HomeBase (HCHB) is preferred
Requirements:
Experience: Minimum of 1 year in Home Health Nursing
Documentation: Proficiency with OASIS is mandatory; HCHB experience preferred
Licensure/Certifications:
Active IL RN license
BLS (AHA)
Skills:
Strong assessment and care planning skills
Excellent communication and interpersonal abilities
Additional: Valid driver’s license and reliable transportation
