Home Health Licensed Practical Nurse (LPN) in Hammond

Responsibilities:

• Deliver comprehensive nursing care to patients in a home health setting
• Assist in conducting patient assessments and developing care plans
• Administer medications and treatments as prescribed
• Monitor and document patient progress and vital signs
• Collaborate with the interdisciplinary team to ensure coordinated care
• Provide education and support to patients and their families
• Maintain accurate and up-to-date patient records

Qualifications:

• Valid and current Licensed Practical Nurse (LPN) license
• Minimum of 6 months of experience in home health care
• HCHB experience preferred
• Strong communication and interpersonal skills
• Compassionate and empathetic attitude towards patients and their families
• Valid driver's license and reliable transportation
