An excellent opportunity awaits an experienced and compassionate Registered Nurse specializing in home health care to provide skilled nursing services in a field-based role. This position will support patients by delivering high-quality, patient-centered nursing care directly in their homes. Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Position Details:

  • Full-time, 13-week contract
  • Caseload of 25–32 points per week (approximately 3–5 patient visits per day)
  • Field-based; daily travel required within assigned area (mileage reimbursement provided)
  • Documentation through Home Care Home Base (HCHB)
  • OASIS tool used for assessments
  • Primary Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized patient care plans
  • Deliver skilled nursing care including wound care, medication administration, IV therapy, and management of chronic conditions
  • Monitor patient progress, recognize and report changes, and coordinate timely communication with the interdisciplinary team and physicians
  • Provide patient and family education on health conditions, treatments, medications, and preventive care
  • Maintain accurate and timely records in HCHB
  • Collaborate with therapy and social work professionals to ensure cohesive care delivery
  • Participate in meetings, case conferences, and ongoing professional education
  • Comply with all applicable regulations and standards
  • Qualifications:

  • Active, unrestricted RN license
  • Minimum one year of recent home health RN experience (field experience preferred)
  • Proficiency with HCHB and OASIS is essential
  • Reliable transportation, valid driver’s license, and auto insurance required
  • Strong independent work ethic, advanced assessment skills, and excellence in communication and organization
  • BSN and experience managing 25–32 points per week strongly preferred
  • Familiarity with Medicare and home health compliance is advantageous
