Deliver essential nursing care directly to patients in their homes as an experienced Home Health Registered Nurse. This full-time, 13-week contract offers the opportunity to make a meaningful difference by fostering patient independence and promoting positive health outcomes through skilled clinical care and compassionate support.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and build individualized care plans.
  • Deliver skilled nursing care, such as wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, recognize and report any changes in condition, and communicate promptly with healthcare providers.
  • Educate patients and families on care plans, medications, and preventive measures.
  • Accurately document care and patient interactions using Home Care Home Base (HCHB).
  • Coordinate with interdisciplinary teams, including therapists, social workers, and home health aides for holistic patient care.
  • Participate in team meetings, training, and case conferences to support consistently excellent care delivery.
  • Ensure all care and documentation meets agency and regulatory standards.

    • Required Qualifications:

  • Active RN license in the state (in good standing).
  • At least 1 year of recent home health field experience.
  • Proficiency with Home Care Home Base (HCHB) and OASIS.
  • Strong clinical assessment, time management, and communication skills.
  • Reliable transportation with a valid driver’s license and auto insurance.
  • Ability to work independently and handle a caseload of 25–32 points per week.

    • Preferred:

  • BSN degree.
  • Experience managing higher-volume caseloads.
  • Familiarity with Medicare/home health compliance.
