Job Title: Travel Home Health RN
Location: Toledo, OH

Type: Travel Contract

Duration: 13 Weeks

Shift: Monday–Friday, with rotating on-call

Job Description:
Sunbelt Staffing is seeking an experienced and dedicated Home Health RN to join a reputable healthcare team in Toledo, OH. This travel contract offers a great opportunity to work independently in the field and help patients manage their health from the comfort of their homes.

Key Responsibilities:

  • Deliver skilled nursing care and health education to patients in their homes

  • Complete thorough assessments and OASIS documentation

  • Utilize Homecare Homebase (HCHB) for all charting

  • Collaborate with physicians and the care team to develop patient-centered care plans

  • Rotate on-call duties as needed

    Qualifications:

  • Active RN license in Ohio

  • 2+ years of recent home health experience

  • Proficiency in OASIS documentation

  • Strong familiarity with Homecare Homebase (HCHB)

  • Current BLS (AHA)

  • Reliable transportation, valid driver’s license, and auto insurance

  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
