Job Description:
Sunbelt Staffing is currently hiring a Home Health Registered Nurse (RN) for a 13-week contract assignment in Somersworth, NH. This position requires proficiency in OASIS documentation and experience with Homecare Homebase software, providing an excellent opportunity to deliver personalized care to patients in their homes while working within a supportive team.

Key Responsibilities:

  • Perform comprehensive patient assessments and accurately document OASIS data
  • Develop individualized care plans tailored to each patient’s needs
  • Administer medications, provide wound care, and monitor patient progress
  • Collaborate with a multidisciplinary team to ensure quality care outcomes
  • Educate patients and families on health management and self-care practices

    Qualifications:

  • Active RN license in New Hampshire
  • Proficiency in OASIS and experience with Homecare Homebase required
  • 2 years of Home Health experience preferred
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
  • Locals and travelers both accepted
