Deliver quality patient care in a dynamic home health environment as a Registered Nurse specializing in field-based services. This 13-week full-time contract prioritizes compassionate, skilled nursing care delivered in patients’ homes using the latest industry standards.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Complete accurate, comprehensive OASIS assessments and develop individualized care plans for each patient.
  • Deliver direct patient care, including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and promptly communicate concerns to physicians and the interdisciplinary healthcare team.
  • Educate patients and families on disease process, medications, treatment plans, and preventive health practices.
  • Document all visits, interventions, and outcomes promptly in Home Care Home Base (HCHB).
  • Collaborate with therapists, home health aides, and social workers to ensure coordinated, holistic care.
  • Participate in case conferences and staff meetings to share insights and contribute to continuous quality improvement.
  • Maintain compliance with agency protocols and regulatory standards for home health delivery.

    • Qualifications:

  • Active RN license (in good standing).
  • At least one year of recent field-based home health RN experience.
  • Proficient with Home Care Home Base (HCHB) and OASIS documentation.
  • Strong clinical assessment, time management, and organizational skills.
  • Self-sufficient and adaptable to managing a flexible field-based assignment.
  • Reliable transportation, current driver’s license, and auto insurance.

    • Preferred:

  • BSN degree.
  • Experience managing caseload expectations of 25–32 points per week.
  • Knowledge of Medicare and home health compliance.
