Home Health, Registered Nurse (RN)-Hermitage, PA

Sunbelt Staffing is seeking a compassionate and dedicated Registered Nurse (RN) to join a Home Health team in the picturesque community of Hermitage, Pennsylvania.

Job Details:

• Position Type: Contract, Full-Time
• Length: 13 Week Contract, possibility of extension upon successful completion and performance
• Weekly Hours: 40 hours per week, Monday to Friday, 8:00 AM - 5:00 PM Additional Availability: Occasional weekends and on-call
• Location: Hermitage, PA
• Benefits: Medical, Dental, Vision, 401k
• Compensation: Hourly pay, not by visit; Negotiable pay, based off territory and experience; Blended pay packages available for travelers; Mileage reimbursement

Job Duties:

• Perform comprehensive assessments of patients' health conditions and home care needs, tailoring care plans to meet each patient's specific goals and preferences.
• Collaborate closely with patients, their families, and the healthcare team to develop individualized care plans focused on maximizing patient independence and well-being.
• Administer medications, provide wound care, and conduct necessary medical procedures with a high level of precision and adherence to best practices.
• Monitor and maintain meticulous electronic records of patients' vital signs, symptoms, and responses to treatment.
• Educate patients and their families on disease management, treatment plans, and preventive measures, emphasizing patient empowerment and self-management.
• Facilitate effective communication and coordination among interdisciplinary healthcare professionals, ensuring comprehensive and seamless patient care.

Requirements:

• Active Pennsylvania RN license
• Active AHA or American Red Cross BLS certification
• Minimum of 1 year of Home Health experience
• Proficiency in Oasis and Homecare Homebase software to be considered for hire
• Reliable transportation
• Ability to pass a drug screen and background check
