Experienced and compassionate Registered Nurses are invited to contribute their clinical skills and patient-centered focus in a flexible, field-based Home Health assignment. This role centers on providing high-quality nursing care to patients in their homes, managing individualized care plans, and promoting optimal health outcomes through seamless collaboration and communication.

Schedule & Setting:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract
  • Average caseload of 25–32 points per week (typically 3–5 patient visits daily)
  • Field-based care with daily travel to homes within an assigned territory

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and create thorough care plans tailored to each patient.
  • Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
  • Evaluate patient progress, monitor for changes in condition, and communicate proactively with care teams and physicians.
  • Educate patients and families on diagnoses, medications, and treatment goals, empowering them toward self-management when possible.
  • Accurately document assessments and interventions using Home Care Home Base (HCHB).
  • Collaborate closely with an interdisciplinary team including therapy, social work, and home health aides.
  • Attend case conferences, staff meetings, and ongoing education as needed.
  • Adhere to current regulatory standards and organizational policies.

    • Desired Qualifications:

  • Active RN license in good standing within the state of practice
  • At least 1 year of recent home health experience (field nursing preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS required
  • Strong assessment, communication, time management, and critical thinking abilities
  • Valid driver’s license, reliable transportation, and up-to-date auto insurance
  • BSN and experience managing Medicare home health compliance are preferred
