Job Description: We are seeking a highly skilled and compassionate Home Health Registered Nurse (RN) with Oasis experience to join our dedicated team in Warwick. As a Home Health RN, you will provide exceptional in-home nursing care to patients, ensuring their well-being and promoting their independence. The ideal candidate will have a strong background in home healthcare, be proficient in using the Oasis electronic documentation system, and possess excellent clinical skills and interpersonal abilities.

Responsibilities:

  • Conduct comprehensive assessments of patients' health status, including physical, psychological, and social needs, in their homes.
  • Develop and implement individualized care plans based on assessment findings, incorporating patients' goals and preferences.
  • Administer medications, treatments, and procedures as prescribed, while maintaining accurate and detailed documentation.
  • Provide skilled nursing interventions, such as wound care, IV therapy, and medication management, in accordance with physician orders.
  • Educate patients and their families on disease management, medication usage, and other health-related topics to promote self-care and independence.
  • Collaborate with interdisciplinary healthcare team members, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Regularly monitor patients' progress, reassessing and updating care plans as needed.
  • Adhere to all regulatory guidelines, including HIPAA and Medicare requirements, and maintain accurate and timely documentation using the Oasis system.

Qualifications:

  • Valid and active Registered Nurse (RN) license
  • Minimum of one year of experience in home healthcare
  • Proficient in using the Oasis electronic documentation system.
  • Strong clinical skills and ability to provide high-quality patient care.
  • Excellent interpersonal and communication skills, with the ability to build rapport and establish trust with patients and their families.
  • Knowledge of Medicare and Medicaid regulations and documentation requirements.
  • Ability to work independently and manage time effectively.
  • Flexibility to adapt to changing patient needs and schedules.
  • Reliable transportation and a valid driver's license.
