Home Health Registered Nurse (RN) – Contract
Location: Amherst, NY
Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Duration: 13 Weeks
Hours: 40 hours/week (No weekends, No call)
Position Overview:
We are seeking an experienced and independent Registered Nurse to provide skilled home health visits throughout the Amherst area. The ideal candidate will be comfortable with case management, timely documentation, and travel within a defined local territory.
Requirements:
Active RN license in New York
Minimum of 1 year of recent home health experience
Strong proficiency with OASIS-E documentation (24–48 hour turnaround)
Experience with HomeCare HomeBase (HCHB) preferred
Reliable transportation and valid driver’s license
Able to manage a full caseload independently
Key Responsibilities:
Conduct start-of-care, routine, and discharge visits
Collaborate with physicians and interdisciplinary team
Develop and revise care plans as needed
Provide patient and caregiver education
Submit documentation accurately and on time
