**Requirements**

- Active and unrestricted RN license in Pennsylvania
- Minimum of one year of experience in Home Health
- Proficiency in homecare/homebase software and Oasis assessments
- Strong knowledge of home health care practices and regulations
- BLS certification preferred

**Responsibilities:**

- Provide skilled nursing care to home health patients
- Conduct comprehensive assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and report any changes to the healthcare team
- Document patient care accurately and timely using homecare/homebase software and Oasis assessments
- Educate patients and their families on proper home care techniques
- Collaborate with interdisciplinary team members to ensure comprehensive patient care
