**Position:** Local/Travel Home Health Hospice RN

**Location:** Bowling Green, Missouri

**Requirements:**

- Active and unrestricted RN license in Missouri
- Minimum of one year of experience in Home Health
- Proficiency in homecare/homebase software
- Strong understanding of hospice care principles and practices

**Responsibilities:**

- Provide skilled nursing care to home health and hospice patients
- Conduct comprehensive assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient conditions and report any changes to the healthcare team
- Document patient care accurately and timely using homecare/homebase software
- Educate patients and their families on hospice care and end-of-life support
- Collaborate with interdisciplinary team members to ensure holistic patient care
