Position: Nurse Practitioner (NP) - SNF Patient Care

Location: Hornell, NY (Primary SNF Location) - Steuben County

Type: Direct Contract - 13 Weeks Assignment

Schedule: Monday through Friday, 8 am - 5 pm

We are seeking a dedicated Nurse Practitioner (NP) for a 13-week assignment in Hornell, NY. The NP will play a vital role in delivering high-quality care to patients in short-term SNF stays (90 days or less) within Steuben County.


  1. Conduct a comprehensive one-time assessment, covering physical examinations, medication reconciliation, medical history reviews, ADLs evaluation, nutritional assessments, cognitive evaluations, pain assessments, fall risk evaluations, and social determinants of health assessments.
  2. Develop and communicate problem lists along with recommended treatment plans for each resident.
  3. Ensure meticulous and complete documentation, including ICD10 coding.
  4. Collaborate effectively with the interdisciplinary care team.
  5. Engage in advanced illness and advanced care planning conversations.

Program Objectives:

The primary goals are to enhance overall care quality, reduce avoidable hospitalizations, and assist members in devising care plans aligned with their aspirations. The NP will work closely with the member s primary care physician, the member/responsible party, and nursing home staff to deliver primary/preventive care and personalized care coordination.


  1. Active NP license in the state of practice. Licensure should be unencumbered, devoid of any ongoing/unresolved disciplinary actions, including probation or practice restrictions.
  2. Minimum 1 year of NP experience.
  3. Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
  4. Reliable transportation to conduct assessments in nursing homes, assisted living, and home settings.
  5. DEA Certification.

Additional Information:

  • Mileage reimbursement between locations at the federal rate.
  • Standard business hours, Monday through Friday (8 am - 5 pm).
