Job Description:
Sunbelt Staffing is hiring an experienced Home Health Registered Nurse (RN) for a 13-week contract in Louisville, KY. This role requires proficiency in OASIS documentation and experience with Homecare Homebase software, providing a valuable opportunity to work in a supportive and patient-centered environment.

Key Responsibilities:

  • Complete thorough patient assessments with OASIS documentation
  • Develop and manage personalized care plans based on patient needs
  • Provide direct nursing care, including wound care and medication management
  • Collaborate with multidisciplinary teams to ensure the best patient outcomes
  • Educate patients and families on effective self-care and health practices

    Qualifications:

  • Active RN license in Kentucky
  • OASIS proficiency and experience with Homecare Homebase required
  • 2 years of Home Health experience preferred
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
  • Locals and travelers both accepted
