Job Title: Licensed Practical Nurse (LPN) - 1:1 Care

Location: Lake Oswego, OR · Tualatin, OR · Beaverton, OR

Type: Full-time, Contract

Schedule: 7:30 AM - 2:30 PM, Monday through Friday

Description:

Soliant Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide dedicated 1:1 care for a client in need of support throughout the day in the Lake Oswego, Tualatin, and Beaverton areas. This role involves providing care both in the home and during transportation to and from school. The ideal candidate will have experience with trach suctioning, managing seizure activity, and handling various medical needs.

Key Responsibilities:

Provide one-on-one nursing care for the client, always ensuring their health and safety.
Assist with trach suctioning and manage associated care requirements.
Support the client with walking and mobility as needed.
Monitor and manage seizure activity, including the potential use of an Ambu- bag if necessary.
Administer and manage g-tube placement and feedings as required.
Assist with diapering and other personal care tasks.
Provide support during transportation, including on the bus to and from school.
Collaborate with the client’s family, school staff, and other healthcare providers to ensure comprehensive care.
Document and report any changes in the client’s condition to the appropriate healthcare professionals.
Ensure all care procedures are carried out in accordance with established protocols and best practices.
Qualifications:

Current and valid LPN license in Oregon.
Proven experience in trach suctioning, g-tube management, and seizure care.
Ability to use an Ambu -bag and perform other emergency interventions as needed.
Experience with diapering and personal care.
Strong communication skills and the ability to work effectively with the client’s family and school staff.
Compassionate, reliable, and able to provide care with a high level of attention to detail.
Ability to handle physical and emotional demands of the job with professionalism.
