Key Responsibilities:

• Collaborate closely with a diverse healthcare team to develop tailored home care treatment plans that meet the unique needs of each patient.
• Maintain accurate and comprehensive records of patient conditions, progress, and treatment plans, ensuring thorough documentation at every stage.
• Administer medications and provide essential wound care with precision and empathy.
• Create a comforting and supportive environment while delivering care to patients in their homes.

Minimum Requirements:

• Active RN licensure in the state of New York.
• At least one year of recent home health experience.
• Current BLS Certification from the American Heart Association.
• Proficiency with HCHB, Oasis, and Outlook is preferred.
• Previous experience managing a home-to-home workload is a plus.

Position Details:

• Embrace fulfilling home-to-home responsibilities within the Lake Katrine, NY area.
• Full-time role, 40 hours per week, scheduled from 8:00 AM to 5:00 PM.
• Mileage reimbursement provided for travel within Lake Katrine.
