Key Responsibilities:

• Collaborate closely with a diverse healthcare team to create personalized home care treatment plans, addressing individual patient needs comprehensively.
• Maintain accurate and detailed records of patient conditions, progress, and treatment plans, ensuring thorough documentation throughout.
• Administer medications and provide essential wound care with precision and compassion.
• Cultivate a supportive and comfortable environment while delivering care to patients within the comfort of their homes.

Minimum Requirements:

• Active RN licensure in the state of Connecticut.
• At least one year of recent home health experience.
• Current BLS Certification from the American Heart Association.
• Proficiency with HCHB, Oasis, and Outlook is desirable.
• Previous experience managing a home-to-home workload is advantageous.

Position Details:

• Embrace fulfilling home-to-home responsibilities within the Trumbull, CT area.
• This is a full-time role, requiring 40 hours per week, scheduled from 8:00 AM to 5:00 PM.
• Mileage reimbursement is provided for travel within Trumbull.
