Are you a Registered Nurse (RN) with a minimum of one year's recent home health experience, exceptional clinical skills, and a heartfelt commitment to providing exceptional patient care? Key Responsibilities:

• Collaborate closely with a diverse healthcare team to develop comprehensive home care treatment plans tailored to individual patient needs.
• Maintain accurate and detailed records of patient conditions, progress, and treatment plans, ensuring thorough documentation at every stage.
• Administer medications and provide essential wound care with precision and empathy.
• Create a comforting and supportive environment while providing care to patients in their homes.

Minimum Requirements:

• Active RN licensure in the state of Kentucky.
• At least one year of recent home health experience.
• Current BLS Certification from the American Heart Association.
• Proficiency with HCHB, Oasis, and Outlook is desirable.
• Previous experience managing a home-to-home workload is beneficial.

Position Details:

• Engage in fulfilling home-to-home responsibilities within the Lexington, KY area.
• Full-time role, 40 hours per week, scheduled from 8:00 AM to 5:00 PM.
• Mileage reimbursement provided for travel within Lexington.
