Key Responsibilities:

• Collaborate with a diverse healthcare team to create comprehensive home care plans.
• Maintain accurate patient records, tracking conditions and progress meticulously.
• Administer medications and provide essential wound care when needed.
• Cultivate a comforting atmosphere while attending to patients in their homes.

Minimum Requirements:

• Active RN licensure in the state of Illinois.
• Minimum one year of recent home health experience.
• Current BLS Certification issued by the American Heart Association.
• Proficiency in HCHB, Oasis, and Outlook.
• Prior experience managing a home-to-home workload is preferred.

Position Details:

• Embrace home-to-home responsibilities within the Galesburg, IL area.
• Full-time role, 40 hours per week, from 8:00 AM to 5:00 PM.
• Mileage reimbursement provided.
