Setting: Nursing Home / Assisted Living Facilities

Sunbelt Staffing is seeking a dedicated Nurse Practitioner to join an ISNP program in Union, NJ. This role focuses on providing patient-centered care in nursing homes and assisted living facilities to improve resident health outcomes.

Key Responsibilities:

Conduct clinical assessments and develop care plans for residents.

Provide on-site primary, preventative, and urgent care.

Monitor high-risk patients and adjust care plans as needed.

Collaborate with primary care physicians and facility staff.

Maintain communication with residents’ families after each visit.

Support smooth patient transitions from hospitals to the facility.

Qualifications:

Active Nurse Practitioner license in New Jersey.

Experience in geriatrics or long-term care preferred.

Strong communication and teamwork skills.
