Job Opportunity: Nurse Practitioner (NP) - Ocean County, NJ

Position Type: 13-week contract with extension potential, Full-time

Location: Ocean County, NJ

Our mission is to deliver proactive care and resources within the comfort of patients' homes, with a goal to minimize hospitalizations whenever appropriate.

Responsibilities:

  • Conduct comprehensive clinical assessments on each resident
  • Facilitate advanced illness and disease trajectory conversations to establish personalized goals of care
  • Develop problem lists and medical treatment plans for each resident
  • Provide primary, preventative, urgent, or acute care on-site
  • Stratify "high-risk" patients to allow for more frequent clinical visits
  • Develop contingency plans for high-risk conditions
  • Communicate and collaborate with residents' primary care physicians
  • Establish lines of communication with responsible parties after each visit
  • Provide routine patient care at assigned facilities, including hands-on bedside care, care plan development, order creation, and collaboration with facility staff, Directors of Nursing (DON), and primary care physicians
  • Conduct single visits to patients transitioning to facilities as part of their hospital discharge plan, assessing patients in the facility and preparing for a safe and healthy transition out of the skilled nursing facility (SNF) (approximately 2-3 visits per day, averaging 90 minutes each)
  • Typically, Nurse Practitioners (NPs) in this role will have a panel of members assigned to their care across multiple nursing facilities (SNF or assisted living facilities [ALF], Independent Living Communities [ILC], etc.).

Requirements:

  • Current Nurse Practitioner (NP) certification and license in the state of New Jersey
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in a geriatric or long-term care setting
  • Proficient in advanced illness management and conducting disease trajectory conversations
  • Excellent communication, interpersonal, and collaboration skills
  • Ability to prioritize and manage multiple responsibilities effectively
  • Familiarity with ISNP or similar healthcare programs is advantageous

Schedule:

  • Monday through Friday, 8:00 am to 5:00 pm
