Job Description:
Sunbelt Staffing is seeking a dependable and experienced Home Health RN for a travel contract in Mansfield, OH. This field-based role involves providing direct care in patients' homes, managing visits independently, and completing documentation via Homecare Homebase.
Key Responsibilities:
Conduct skilled nursing visits and patient education in the home
Complete OASIS documentation accurately and on time
Chart using Homecare Homebase (HCHB) software
Collaborate with interdisciplinary teams to ensure quality care
Participate in the on-call rotation as scheduled
Qualifications:
Active Ohio RN license
Minimum 2 years of recent home health experience
Strong proficiency in OASIS and Homecare Homebase (HCHB)
Current BLS (AHA) certification
Reliable transportation, valid driver’s license, and auto insurance
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
