Part Time NP Job - Home Health- Flexible Schedule- Tulsa, OK

Sunbelt is looking to hire a Part Time Nurse Practitioner Home Health in Tulsa in the Tulsa County of Oklahoma. The Part Time NP will conduct home health risk assessments. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in the assigned area. Must have 6 months NP experience and/or Home Health experience as an RN prior.

Work Hours:
Must commit to at least 10-40 hours a week
Flexible shifts between Monday and Sunday
Create your own schedule
Minimum weekly or bi-weekly hourly commitment

Responsibilities:
Conduct annual home health risk assessments for a Medicare Advantage health plan
Evaluate prior and current medical history, medical conditions and medication history
Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
Provide education on medical conditions and medications

Preferred Qualifications:
Nurse Practitioner License
Current CPR certification (BLS)
Board Certification through AANP or ANCC
Education Requirements:
MSN degree from an accredited school of nursing

Preferred Experience:
1 year of current NP experience is preferred
