Job Duties:
Perform comprehensive assessments to evaluate patients' physical, emotional, and spiritual needs in the home setting
Develop and implement individualized hospice care plans focused on comfort, dignity, and quality of life
Administer pain management and palliative treatments in accordance with the hospice plan of care
Provide compassionate education and emotional support to patients and families regarding end-of-life care and symptom management
Maintain accurate and timely documentation of patient status and communicate updates with the interdisciplinary hospice team to ensure coordinated care
