Responsibilities:

Provide compassionate and high-quality nursing care to patients in their homes.
Conduct comprehensive assessments of patients' health status and develop individualized care plans.
Administer medications, treatments, and wound care as prescribed by the physician.
Educate patients and their families on disease management, medication administration, and self-care techniques.
Collaborate with interdisciplinary healthcare team members to ensure coordinated and holistic patient care.
Maintain accurate and timely documentation of patient assessments, interventions, and outcomes.
Requirements:

Active LPN license in Illinois 
Minimum of 1-2 years of recent experience working as an LPN in a home health setting.
Strong clinical skills and the ability to work independently in a fast-paced environment.
Excellent communication and interpersonal skills.
Valid driver's license and reliable transportation.
