Description: Join a dynamic 12-month travel contract as a Registered Nurse (RN) in Bronx, NY. This position offers the unique opportunity to work across two facilities, providing care Monday through Friday with flexible scheduling options. COVID-19 vaccination is mandatory for this role.

Responsibilities:

  • Conduct routine monthly visits as directed by the Nurse Practitioner (NP)
  • Evaluate residents' statuses and aid in achieving progress towards their healthcare goals
  • Represent the NP at meetings and relay pertinent updates to the Interdisciplinary Care Plan (ICP) via the Interdisciplinary Care Team (ICT)
  • Coordinate care activities within the skilled nursing facility (SNF) to ensure residents' needs are comprehensively addressed
  • Assist in addressing clinical and quality concerns, including identifying and resolving gaps in care provision
  • Foster effective communication and engagement with residents, their families, SNF staff, and other members of the interdisciplinary care team
  • Maintain regular communication with the Advanced Practice Clinician (APC) regarding resident status and facility requirements
  • Participate in resident rounds and contribute to Advance Care Planning (ACP) initiatives as required
  • Collaborate full-time with a single or group of NPs, supporting their members and facilitating care coordination within the facilities

Requirements:

  • Active RN license in the state of New York
  • Current BLS certification from the American Heart Association (AHA) or American Red Cross (ARC)
  • Minimum of one year of experience as a Registered Nurse (RN)
