Deliver compassionate care as a Home Health Registered Nurse, where your clinical expertise and dedication impact patients’ lives while they heal at home. This full-time, 13-week engagement offers a rewarding opportunity to support patients with individualized, high-quality nursing services in a dynamic, patient-centered environment.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • What You’ll Do:

  • Complete comprehensive OASIS assessments and develop tailored care plans.
  • Deliver skilled nursing interventions: wound care, medication administration, IV therapies, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate proactively with care teams and physicians.
  • Educate patients and their families on disease processes, medications, and health management.
  • Document timely and accurately using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides to ensure comprehensive care.
  • Attend case conferences, contribute to team meetings, and participate in ongoing education.
  • Travel daily to provide care within your assigned territory (mileage reimbursement provided).

    • Key Qualifications:

  • Active RN license in good standing.
  • At least 1 year recent Home Health field experience.
  • Proficiency in HCHB and knowledgeable in OASIS documentation.
  • Strong clinical assessment, communication, and organizational skills.
  • Reliable transportation, valid driver’s license, and auto insurance.
  • Ability to work autonomously and manage a field-based caseload of 25–32 points per week (typically 3–5 visits daily).

    • Preferred:

  • BSN degree.
  • Previous experience in managing high patient caseloads.
  • Knowledge of Medicare and compliance in home health care.

    $1,709 - $1,807/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
