Schedule:

In this field-based position, you will conduct OASIS assessments, craft individualized care plans, and play a vital role in the recovery and well-being of patients. Utilizing the Home Care Home Base (HCHB) system, you’ll document care and ensure compliance with all regulatory standards. Daily travel is required to patient homes within your assigned territory, with mileage reimbursement provided.
