Schedule:

  • Flexible shifts available Monday through Sunday
  • Customize your own work schedule
  • Minimal commitment required: choose weekly or bi-weekly availability
  • Primary Responsibilities:

  • Perform annual risk assessments in patients’ homes
  • Evaluate prior and current medical history, health conditions, and medications
  • Educate patients on disease management and proper medication usage
  • Identify gaps in care and provide actionable recommendations
  • No requirement to create treatment plans or prescribe medications
  • Desired Skills and Qualifications:

  • Active Texas Nurse Practitioner license
  • MSN degree from an accredited nursing program
  • Board certification through AANP or ANCC
  • CPR certification (BLS)
  • Preferably at least 6 months of current NP experience, particularly in home health
  • Strong interpersonal skills and ability to adapt to varied home settings
