As a Nurse Practitioner, you will visit Medicare Advantage members, conducting comprehensive annual and post-discharge evaluations in settings such as Rupert, Heyburn, and Paul, Idaho. Each assessment typically takes about 45 minutes, giving you the opportunity to engage directly with members in their homes, provide crucial education, and bridge any gaps in their care—without the pressure of prescribing medications or developing treatment plans.

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review and document prior and current medical history, conditions, and medications
  • Empower patients with education on their health and medication use
  • Identify gaps in care and offer recommendations for better health outcomes
  • Focus solely on assessment—no treatment planning or prescribing required
  • Qualifications

  • Active Nurse Practitioner license in Idaho
  • Current BLS (CPR certification)
  • Board Certified by AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least six months of nurse practitioner experience preferred
