Make a meaningful impact by delivering essential home health risk assessments for Medicare Advantage plan members in Shoshone County, Idaho. Per diem opportunities are available for experienced Nurse Practitioners to perform comprehensive home visits, supporting patient well-being and care coordination. Visits are typically 45 minutes each and may involve annual health assessments or post-discharge evaluations in various locations such as Kellogg, Pinehurst, and Osburn.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Set your own schedule based on your availability
  • Commit to a minimum number of hours weekly or bi-weekly
  • Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current health conditions, and medication regimens
  • Educate members about their health conditions and medications
  • Identify care gaps and provide actionable recommendations to promote better outcomes
  • This role does not include creating treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS required)
  • Board Certification via AANP or ANCC
  • At least six months of current NP experience preferred
  • MSN from an accredited nursing program
  • Your commitment to quality and patient education will be compensated at $80-$90 per evaluation, in addition to a mileage stipend and payment for administrative time.
