Make a meaningful impact on patient care by providing in-home health assessments as a Nurse Practitioner in Monroe County. This per diem opportunity offers the flexibility to create your own schedule, allowing you to balance your professional and personal commitments effectively. You’ll be conducting 45-minute risk assessments for Medicare Advantage plan members in areas including Key West, Key Largo, Marathon, and surrounding Monroe County communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own hours and build a schedule that works for you
  • Minimum weekly or bi-weekly hourly commitment based on your availability
  • Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage program participants
  • Review medical history, evaluate current conditions, and assess medication lists
  • Provide patient education on diagnosed medical conditions and prescribed medications
  • Detect care gaps and recommend follow-up services or interventions as needed
  • No requirement to create treatment plans or prescribe medications
  • Qualifications:

  • Active Nurse Practitioner license in Florida
  • Completed MSN degree from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred, but not required
  • Compensation includes $80-$90 per evaluation, with additional mileage and administrative time stipends.
