A rewarding opportunity is available for a Nurse Practitioner with home health experience to conduct at-home risk assessments for Medicare Advantage plan members throughout Clark County, WA. This per diem role offers flexibility, autonomy, and the chance to make a direct impact on patients in Vancouver, Camas, Battle Ground, and the surrounding neighborhoods.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own work schedule
  • Choose minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments, typically lasting about 45 minutes, for Medicare Advantage members
  • Review prior and current medical history, assess current medical conditions, and medication use
  • Offer patient education on health conditions and prescribed medications
  • Identify and document any gaps in care, providing actionable recommendations to support care coordination
  • No development of treatment plans or medication prescribing is required
  • Qualifications:

  • Active Nurse Practitioner license in Washington (WA)
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited nursing program
  • Minimum of 6 months of nurse practitioner experience preferred
  • Enjoy competitive earnings of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
