As a Nurse Practitioner specializing in home health, you’ll support patients in Clallam County through proactive evaluations—empowering them with knowledge and providing valuable insights for their ongoing care.

Schedule & Flexibility:

  • Flexible shifts offered seven days a week
  • Ability to set your own schedule
  • Minimum weekly or bi-weekly hours to fit your availability
  • Key Responsibilities:

  • Conduct annual and post-discharge risk assessments in individual patient homes
  • Assess prior and current medical history, conditions, and medication profiles
  • Deliver education to patients regarding their health conditions and prescribed medications
  • Identify care gaps and offer clinical recommendations to address them
  • No ongoing treatment planning or medication prescribing required
  • Qualifications:

  • Active Nurse Practitioner license in Washington State
  • Current CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • MSN degree from an accredited nursing program
  • Compensation ranges from $80 to $90 per evaluation, and includes mileage stipends and pay for administrative time—recognizing the importance of your commitment both on the road and in documentation.
