Experienced Nurse Practitioners are invited to support home health initiatives within Lewis County, WA, on a per diem basis. In this dynamic role, you will perform comprehensive home health risk assessments for Medicare Advantage plan members residing in Centralia and surrounding areas such as Chehalis, Morton, Mossyrock, and more. Each risk assessment averages about 45 minutes and may include annual health assessments or post-discharge visits.

Schedule and Location Highlights:

  • Flexible shift availability between Monday and Sunday
  • Self-scheduling—set your own hours
  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical histories, conditions, and medications
  • Educate patients about their health conditions and medication regimens
  • Identify care gaps and make recommendations to close them
  • No requirement to create treatment plans or prescribe medications
  • Preferred Qualifications:

  • Current Nurse Practitioner License in Washington
  • Active CPR (BLS) certification
  • Board Certification through the AANP or ANCC
  • 6 months or more of recent NP experience preferred
  • Master of Science in Nursing from an accredited program
