This role focuses on serving Medicare Advantage plan members throughout Kay County, OK—including Ponca City, Blackwell, Newkirk, and Tonkawa. Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitments
  • Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in the community
  • Review prior/current medical and medication history with patients in home settings (each evaluation averages 45 minutes)
  • Educate patients on existing medical conditions and medication use
  • Identify gaps in care and provide recommendations for follow-up or resolution
  • Document clinical findings accurately (no treatment plan development or medication prescribing required)
  • Desired Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Board certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • Minimum of 6 months’ recent NP experience preferred
  • MSN from an accredited school of nursing
  • Enjoy highly competitive pay of $80-$90 per completed evaluation, plus mileage stipend and compensation for administrative time.
