Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in various Fayette County locations
  • Review prior/current medical histories, medical conditions, and medication usage
  • Educate patients on their health conditions and medication regimens
  • Identify care gaps and make recommendations for improved outcomes
  • Focus solely on assessments—treatment planning and prescribing are not required
  • Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Minimum of 6 months’ NP experience preferred
  • MSN from an accredited nursing school
  • This position offers attractive per evaluation compensation between $80-$90, plus a mileage stipend and compensation for administrative time.
