The average assessment lasts about 45 minutes and may involve annual health reviews or post-discharge follow-ups.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical histories, existing conditions, and medication profiles
  • Educate patients on their health conditions and medications
  • Identify gaps in healthcare and make recommendations to support patient well-being
  • No requirement to create treatment plans or prescribe medications
  • Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited nursing school
  • At least six months of nurse practitioner experience preferred
  • This role offers an engaging and autonomous practice environment with the opportunity to interact directly with patients across Jerome County, including towns such as Jerome, Hazelton, and Eden.     Compensation for this role provides $80-$90 per completed evaluation, along with a mileage stipend and additional compensation for administrative activities.
